Senior Health Connection, 7903 Providence Rd., Charlotte To help coordinate senior care, Carolinas HealthCare System has created Senior Health Connection. This Charlotte-based, Carolinas Physicians Network practice is dedicated to preventive care, the diagnosis and treatment of dementia, as well as the management of chronic diseases. We use a team approach to coordinate care as patients and their caregivers move through changes in the body, mind and spirit. Our team includes two physicians who specialize in geriatrics. We also provide family education classes and information about disease management and community resources. Patients and families can call for consultations and appointments. Physicians can refer patients to our practice and will be kept up to date with the treatment plan. We accept Medicare, Medicaid and private medical insurance.

It's a myth that we need less sleep in our senior years.

Steps to make your home safer for seniors:

Provide good lighting throughout the house, including night-lights.

Tape down area rugs or remove them if possible.

Rearrange furniture to prevent blocked exits.

Install a smoke detector and a carbon monoxide detector on every floor of the house, including the basement.
Facts about osteoporosis:

1 out of every 2 women and 1 out of every 8 men older than 50 will have an osteoporosis-related fracture in their lifetime.

80% of those affected by osteoporosis are women.

White women age 60 and older have at least twice the incidence of fractures as African-American women.

1 out of 5 African-American women are at risk of developing osteoporosis.
