QUALITY MANAGEMENT PROGRAM

Vision
CMC-Home Care strives to be a quality, alternate site home healthcare provider by consistently meeting clients' medical needs in the most cost-effective manner and by using the highest standards of excellence in patient care.

Objectives
Ensure that the highest quality of care is provided by CMC-Home Care and CMC-Home Infusion and Equipment. Collect, screen and evaluate information in order to identify and pursue opportunities to improve all important functions and to achieve a high level of client satisfaction; to understand and maintain stability in systems and processes. Identify, track and improve process in client care, services and satisfaction to ensure improvement. Meet state and federal regulatory requirements, JCAHO accreditation standards for quality management and other contract requirements.

Methods
Outcomes
CMC-Home Care collects data that measures organizational and program specific outcomes. Our focus is on meeting therapy goals and in monitoring adverse outcomes such as infections, acute hospital admissions and adverse drug interactions. These outcomes can delay patient progress and increase healthcare costs. Our goal is to assure our processes prevent or proactively recognize adverse outcomes.

Process Measures
Data is also collected to allow CMC-Home Care to evaluate the effectiveness, timeliness, cost and outcomes of the processes we use to provide care and services. These include evaluating the match between patients' needs and skilled staff selection to optimize providing the most appropriate and cost-effective skill level to meet patients/caregiver needs. The goal of our process measures are to make sure we are doing the right thing at the right time for the right patient with the right care provider and the right equipment and supplies.

Utilization Data
CMC-Home Care monitors resource utilization including the number of visits per discipline specific to top admission diagnoses and high cost diagnosis.
