CMC-Home Care is a full service, alternate site healthcare facility that is an integral part of Carolinas HealthCare System, the premier healthcare provider in North Carolina. Scope of Service
CMC-Home Care provides a broad range of healthcare services in the comfort and convenience of the home setting or in an alternate care site. Our highly trained staff can meet virtually all home care needs. Our services include:

Intake/Referral/Case Management
In order to promote a more efficient referral and intake process, CMC-Home Care has implemented a centralized referral intake system. The goal of this system is to provide a single contact person for referral sources at the time of referral and a case manager to coordinate care for their clients and to provide ongoing communication and follow-up related to billing and clinical issues.

Telephone referrals are received through our referral center and are routed to the appropriate intake area. The intake area consists of a team of clinical, financial and clerical staff who are able to provide internal case management and monitor patient care/services in order to ensure quality of care and cost effectiveness.

Registered nurses can assist you with the following services:

  • Act as the primary contact person for referrals that require skilled nursing, infusion and/or rehabilitation services.
  • Obtain case management and authorization for all services for patients whose insurance requires this to be done.
  • Maintain ongoing communication with all appropriate payer sources, physician offices, patients and field staff to support timely reimbursement and facilitate coordination of services.
  • Provide case management for those patients without any identified payer source.
Patient account representatives can assist you as follows:
  • Work with clinical staff to ensure that the dates of service and documentation support insurance provider requirements.
  • Work with clinical staff to manage patients to maximize cost savings.
  • File accurate and timely insurance claims for skilled services that are provided to patients whose referrals are processed by the team.
  • Work with the team to identify the most efficient and cost-effective way to provide care to patients.
  • Function as a contact person for inquiries from patients, clinical staff, referral sources and insurance providers regarding the status of claims.
  • Provide electronic filing options as available.
