Maintaining health and independence is important in any stage of life. For seniors, a simple sprained ankle from a tennis match can become complicated. Senior Health Connection is the right partner for the many health challenges seniors may need to tackle.
Senior Health Connection exists to help people over the age of 60 improve their health and quality of life, maintain independence and maximize their functional ability. The program focuses on diagnosis, treatment and preventive healthcare with the goal of avoiding illness, discomfort and unnecessary hospitalization.
Our team of board-certified geriatricians, nurse practitioners, registered nurses, a clinical pharmacist and a social worker unite with patients and their caregivers to find the right solutions or support. Senior Health Connection is dedicated to primary as well as consultative care; diagnosis and treatment of dementia; management of chronic disease; and treatment of acute illnesses. The team also provides patient and family education regarding disease management, geriatric care management and information on community resources.
