GUIDELINES FOR PERSONAL STATEMENT

Guidelines for Personal Statement
Please answer the following questions within the text of your personal statement to help us know you and your special interest. Also, please let us know if you would like to be considered for an interview at the main program, urban site, and/or rural program. You may apply to 1, 2 or 3 of these programs.

  1. What is it that interests you in the residency program at Carolinas Medical Center in Charlotte, North Carolina?
  2. What led you to choose family medicine as a career and what valuable lesson have you learned through your experiences in family medicine to date?
  3. What Leadership roles and personal accomplishments have you achieved through the years?
  4. What are your future goals in a career of family medicine?

Applicants to the Urban Site
Describe any specific interest or experience working with the urban underserved or ethnically diverse populations. Please note, the Urban Site has a separate ERAS address.

Applicants to the Rural Program
Describe your interest in training in a rural program - outside a traditional medical education setting, especially in a program located in a small town. Please note, the Rural Program has a separate ERAS address.

Couples Match
If you are applying with another residency candidate in the couples match, please provide his or her name and medical specialty on your application.
